HIFU Treatment: A Complete Guide

“What exactly is HIFU?”
“I’d like to know about HIFU’s effects, potential side effects, and how the treatment works.”

If you have questions like these, this page is for you.

HIFU is a body-contouring treatment that uses focused ultrasound heat to firm the skin from within, without damaging the skin’s surface. It may offer cosmetic benefits such as improving facial sagging and lifting the contours of the face.

With minimal downtime, it is possible to firm areas of concern without injuring the skin.

On this page, we explain HIFU’s effects, potential side effects, and the different types of HIFU devices available.

At our clinic, we propose a treatment approach suited to each individual patient and focus on delivering results.
We take the time to carefully listen to your concerns and symptoms, so please book a consultation using the button below.

What Is HIFU? An Ultrasound Therapy with Potential Cosmetic Benefits

HIFU stands for High Intensity Focused Ultrasound, a treatment method that delivers concentrated ultrasound energy to targeted layers beneath the skin.

It is one of the “lifting” approaches used to address wrinkles and sagging, and works by using ultrasound to improve skin laxity. Ultrasound energy is directed at the subcutaneous tissue and fascial layers, where the resulting heat firms and lifts the skin from within.

How HIFU Works | Firming the Skin from Within Using Heat, Without Burning the Surface

HIFU firms the skin from the inside using heat, without burning the skin’s surface.

The mechanism of action is as follows:

  1. Ultrasound energy is delivered to the target area
  2. The skin is firmed and lifted
  3. Production of collagen and elastin is stimulated

HIFU precisely focuses ultrasound energy on the intended skin layer. It can deliver intense thermal energy to the target layer while minimizing impact on the surrounding tissue and the skin’s surface.

This thermal energy helps to firm and lift the face.

Furthermore, the skin tissue that receives the heat responds by stimulating the production of collagen and elastin—key components that contribute to skin firmness—helping to promote more resilient-looking skin alongside the lifting effect.

Medical HIFU vs. Esthetic HIFU | Different Delivery Methods

HIFU is available in two forms: medical HIFU and esthetic (salon) HIFU. Please refer to the table below for a comparison of the two.
※ Esthetic HIFU is not approved by government authorities, whereas medical HIFU is a government-approved procedure.

TypeMedical HIFUEsthetic HIFU
Delivery MethodFocused point deliveryThermal accumulation delivery
Features・Higher output
・Thermal energy reaches deeper skin layers
・Greater potential for firming and lifting results
・Lower output
・Thermal energy only reaches shallower skin layers
・Limited effectiveness
DiscomfortMinimalNone
Treatment FrequencyOnce every 3–6 months1–2 times per month

Medical HIFU and esthetic HIFU differ in their delivery method and output level.

Medical HIFU operates at a higher output level and may offer greater results compared to esthetic HIFU.

Medical HIFU also starts at a lower output and is gradually adjusted, so discomfort during treatment is typically minimal. If you have concerns about pain, please discuss this with your treating physician beforehand.

Who May Benefit from HIFU | Those Concerned About Facial Wrinkles and Sagging

HIFU treatment may be worth considering if any of the following apply to you:

  • You are bothered by nasolabial folds or marionette lines
  • You would like to address fullness or sagging along the jawline and cheeks
  • You are concerned about a double chin
  • You are looking for a lifting option with minimal pain and risk

HIFU can address a wide range of areas on the face, so if you are concerned about facial wrinkles or sagging, please feel free to consult with us.

What Results Can HIFU Offer? Three Potential Cosmetic Benefits

HIFU treatment may offer three main cosmetic benefits:

  • Improvement of nasolabial folds and sagging around the eyes and eyelids
  • A slimmer facial appearance through jawline improvement
  • Slimming of the abdomen and firming of the upper arms

Below, we explain each benefit in more detail.

Benefit ① Improvement of Nasolabial Folds and Sagging Around the Eyes and Eyelids

HIFU may help improve the appearance of nasolabial folds and sagging around the eyes and eyelids.

Nasolabial folds are primarily caused by facial sagging.

When the facial muscles weaken or collagen in the dermis decreases, the dermal layer becomes compromised. This is thought to lead to a loss of firmness and elasticity, a reduction in the muscle support holding up the cheeks, and ultimately the formation of nasolabial folds.

HIFU works by delivering energy that causes the tissue from the dermal layer to the fascial layer to contract, helping to address facial sagging and the appearance of nasolabial folds.

Benefit ② A Slimmer Facial Appearance Through Jawline Improvement

HIFU may also be beneficial for improving the jawline, including sagging under the chin and a double chin.

One of the contributing factors to a sagging facial appearance is laxity in the SMAS layer, which lies between the subcutaneous tissue and the facial muscles. The SMAS layer, which plays a role in supporting the skin, can deteriorate and loosen with age.

Over time, it becomes less able to support the facial skin, leading to sagging and wrinkles.

HIFU works to firm the SMAS layer and address sagging from within the skin. By improving jawline laxity, it may help refine the facial silhouette and contribute to a slimmer-looking face.

Benefit ③ Slimming the Abdomen and Firming the Upper Arms

HIFU may also be effective for slimming the abdomen and firming the upper arms.

When HIFU is applied to areas of the body other than the face, it is referred to as “HIFU Body,” and involves directing thermal energy at individual fat cells. Fat cells that are no longer functioning are recognized as waste by the body and naturally eliminated.

It is generally considered that fat cells are expelled from the body approximately 2–3 weeks after treatment, with a gradual reduction in size at the treated area experienced over time.

In addition to the abdomen and upper arms, targeted slimming of various areas such as the waist and buttocks may also be possible.

How Long Do HIFU Results Last? Around 3–6 Months as a General Guide

There is no fixed limit on how long HIFU results last. The fascia that has been firmed through HIFU generally remains tightened.

However, as the natural aging process continues, sagging will gradually return over time, meaning the face may slowly begin to sag again as time passes.

As a result, you may start to notice sagging returning approximately 3–6 months after your HIFU treatment.

Does HIFU Really Work? When Will Results Appear? Results May Take Some Time to Become Noticeable

The time it takes for results to appear varies from person to person.

The lifting effects of HIFU emerge gradually over time. Some patients notice results soon after treatment, and in many cases effects become noticeable within one month of the procedure.

For approximately three months following treatment, the skin’s internal structures continue to produce beneficial components, meaning the skin gradually lifts and firms during this period.

Advantages of HIFU | Short Treatment Time

The main advantages of HIFU include the following three points:

  • Short treatment time
  • No visible marks or scarring from the procedure
  • Minimal burden on the skin

HIFU treatment covering the entire face takes approximately 40 minutes on average, making it easy to fit into a busy schedule.

The procedure leaves no marks on the skin and places minimal stress on it. By adjusting the number of shots according to the area you wish to address, treatment can be tailored to your individual concerns.

Disadvantages of HIFU | Some Discomfort During Treatment

The main disadvantages of HIFU include the following three points:

  • Not suitable for those who are pregnant or breastfeeding
  • Risk of side effects
  • Increased sensitivity to UV exposure after treatment

HIFU is not suitable for patients who are pregnant or breastfeeding. Please also note that individuals with a pacemaker or those who have recently undergone other cosmetic treatments may not be eligible for HIFU.

In addition, HIFU treatment may temporarily reduce the skin’s moisture levels, leading to dryness. As a result, the skin’s ability to protect itself from UV-related damage may be temporarily reduced, making it more susceptible to sun exposure.

After HIFU treatment, be sure to keep the skin well moisturized and take adequate sun protection measures.

Potential Side Effects of HIFU | Redness and Swelling of the Face

Possible side effects that may occur following HIFU treatment include:

  • Redness
  • Swelling
  • Welts on the skin’s surface
  • Scabbing
  • Bruising

and others.

Downtime and side effects are generally minimal; however, in rare cases, side effects such as redness or swelling may occur. These symptoms are typically temporary and tend to resolve on their own over time.

If side effects are severe or persist, please consult your clinic.

The HIFU Treatment Process

HIFU treatment at our clinic follows the steps below.

  1. Initial Consultation & Counseling
We begin by listening carefully to your concerns. We take the time to understand each patient’s individual goals—such as reducing sagging around the eyes or achieving a lifting effect—and work together to decide on a treatment plan.
  2. Treatment
The procedure is performed to address your specific concerns. If you are worried about discomfort during treatment, please discuss this with your physician beforehand.
  3. Aftercare
We recommend planning your next visit approximately 3–6 months after treatment. Scabbing may occasionally occur following the procedure, but this typically resolves over time.

Makeup and normal activities are permitted from the day of your HIFU treatment. However, please take care not to rub the treated area firmly.

How Often Should HIFU Be Done? Once Every 3–6 Months as a General Guide

The appropriate frequency and interval for HIFU treatment varies from person to person. As a general guideline, once every 3–6 months is recommended.

Collagen production at the treated site is stimulated within approximately one month of treatment, helping to maintain results.

For specific guidance on treatment intervals, please consult with a physician at your clinic visit. At our clinic, we propose a treatment frequency and approach suited to each individual patient’s condition.

Recovery and Downtime After HIFU | Swelling May Appear the Following Day

After HIFU treatment, swelling may begin to appear from the following day. The duration of swelling varies among individuals, but 2–7 days is a typical timeframe.

On the day of treatment, be sure to apply thorough sun protection and keep the skin moisturized. While any soreness similar to muscle aches persists, avoid applying strong pressure or massaging the treated area.

Showering and bathing are permitted from the day of treatment. However, if redness is present at the treated site, it is advisable to avoid bathing on that day and wait until the following day or later.
