Ketoconazole: What Does It Do? A Guide to the Different Forms — Cream, Lotion, and Shampoo — and How to Use Them

“What conditions can ketoconazole help with?”
“Do ketoconazole cream, lotion, and shampoo serve different purposes?”

If you have questions like these, read on.

Ketoconazole is a prescription antifungal medication. It may help treat seborrheic dermatitis and tinea (athlete’s foot), and it has also shown potential benefits in AGA (androgenetic alopecia) treatment.

However, ketoconazole is not expected to promote new hair growth. If your goal is to address hair thinning, AGA treatment medications are recommended instead.

This page explains ketoconazole’s effects, potential side effects, and how to use each formulation — cream, lotion, and shampoo.

At our clinic, we propose a treatment approach tailored to each patient’s individual needs and focus on delivering results.

What Is Ketoconazole? A Prescription Medication for Improving Scalp Condition

Ketoconazole is a prescription antifungal medication. Its active ingredient is ketoconazole itself, and it is available in formulations such as “Ketoconazole Cream 2%” and “Ketoconazole Lotion 2%.”

Fungi are mold-like organisms that can cause skin conditions. When the scalp environment is compromised, fungi can proliferate, leading to dandruff, itching, and inflammation.

Ketoconazole works by inhibiting fungal cell membrane synthesis, eliminating fungi and helping to improve the scalp environment.

Differences Between Ketoconazole Cream (Ointment) and Lotion

Ketoconazole is available in different formulations, each with distinct characteristics and recommended application sites.

Formulation Characteristics Application Site
Cream
(Ointment)
· Tends to feel greasy
· Lower penetration
Face, underarms, genital area, etc.
Lotion · Light, non-greasy texture
· Higher penetration
Scalp

Because cream tends to feel greasy, it is not well suited for use on the scalp. Cream (ointment) formulations are therefore used for skin infections such as tinea, candidiasis, and acne.

Lotion, on the other hand, has higher penetration and is not suitable for application to open wounds or broken skin. Its light texture makes it appropriate for application to the scalp in the treatment of seborrheic dermatitis and AGA.

In addition to cream and lotion, ketoconazole is also manufactured and available in spray form.

Ketoconazole as a Generic Version of Nizoral

Nizoral is a medication that contains ketoconazole as its active ingredient.

Nizoral is the brand-name (originator) drug, and ketoconazole products are its generic equivalents.

There are no differences in efficacy, indications, or side effects between Nizoral and generic ketoconazole. However, generic ketoconazole is generally lower in price, making it more accessible.

Effects and Indications of Ketoconazole | Seborrheic Dermatitis, Acne, and Tinea

Ketoconazole may be effective for the following conditions:

  • Seborrheic dermatitis
  • Acne
  • Tinea pedis (athlete’s foot)
  • Pityriasis versicolor (tinea versicolor)
  • Tinea cruris (jock itch)
  • Tinea corporis (ringworm)

The primary cause of seborrheic dermatitis is believed to be a fungus called Malassezia.

Ketoconazole has strong antifungal activity against Malassezia and may help improve seborrheic dermatitis of the scalp.

Tinea pedis, tinea cruris, and tinea corporis are also caused by dermatophytes (tinea fungi), which are another type of fungus. Applying ketoconazole may help address these conditions as well.

Can Ketoconazole Help with AGA? Potential to Inhibit 5α-Reductase

Ketoconazole may also offer benefits in AGA treatment.

AGA (androgenetic alopecia) is a form of male-pattern hair loss triggered by an increase in the male hormone dihydrotestosterone (DHT). DHT is produced when the male hormone testosterone binds with an enzyme called 5α-reductase.

Because ketoconazole has the potential to inhibit 5α-reductase activity, it may help slow the progression of hair thinning associated with AGA.

The Japanese Dermatological Association has also recognized the potential value of ketoconazole in AGA treatment, noting that “topical ketoconazole may be used.”

Does Ketoconazole Work? It Is Not Expected to Promote Hair Growth

Some people worry that ketoconazole may not be effective for hair thinning.

Please note that ketoconazole is not expected to stimulate new hair growth or promote hair regrowth.

The benefits of ketoconazole are to help maintain a healthier scalp environment and to potentially slow the progression of hair thinning caused by AGA.

If your goal is to promote hair growth, a treatment containing minoxidil as the active ingredient is recommended. Please refer to the following page for a detailed explanation of minoxidil.

Ketoconazole Cream May Also Help with Candidiasis — How to Apply to the Genital Area

Ketoconazole cream may also be beneficial in the treatment of candidiasis.

Candidiasis is a skin condition that tends to occur when the body’s resistance is lowered. Symptoms can include itching.

Candidiasis can affect the genital area. In the case of male external genitalia, ketoconazole is applied to the affected area once daily as part of treatment.

In women, candidiasis can develop inside the vagina. Ketoconazole is rarely used for intravaginal treatment. Treatment with antifungal vaginal suppositories (medications inserted into the vagina) is the general approach.

How to Use Ketoconazole — By Formulation Type

Ketoconazole is available in cream, lotion, and shampoo formulations.

The following sections provide detailed instructions on how to use each type.

How to Apply Ketoconazole Cream and Lotion

The basic method for using ketoconazole cream or lotion is to apply it to the affected area once daily. For seborrheic dermatitis, however, it should be applied twice daily.

Follow the prescribed frequency and apply it daily for the duration advised by your doctor. Do not apply more than instructed even if improvement seems slow.

When applying ketoconazole to the face, rinse immediately with water if it gets into your eyes.

How to Use Ketoconazole Shampoo

Please follow these steps when using a ketoconazole-containing shampoo:

  1. Massage the shampoo into the hair and scalp thoroughly.
  2. Leave it on for 3–5 minutes, then rinse out thoroughly with warm water.
  3. Apply conditioner or treatment to the ends of your hair.
  4. Gently towel dry your hair and finish with a hair dryer.

When using ketoconazole shampoo for AGA treatment, use it daily for the first two weeks, then reduce to 2–3 times per week thereafter.

If you notice dryness or your hair feeling rough after using the shampoo, try using a conditioner or hair treatment alongside it. If the issue persists, consult your doctor.

Ketoconazole Lotion Can Be Mixed into Regular Shampoo

As mentioned above, ketoconazole shampoo is one option, but it is also possible to mix ketoconazole lotion into your regular shampoo.

A general guideline is to mix one bottle of ketoconazole lotion into 100 ml of shampoo. However, not all shampoos are suitable for mixing with ketoconazole, so please consult your doctor in advance.

Note that while ketoconazole-containing shampoos are available in some countries, they are not approved for sale in Japan.

Can Ketoconazole Be Used Together with Steroids? Using Each According to Your Condition

Ketoconazole and steroids should not be used at the same time. While steroids may be used in the treatment of seborrheic dermatitis, the general approach is to alternate between them depending on the condition.

Steroids have a strong anti-inflammatory effect and may be used temporarily when seborrheic dermatitis symptoms are severe. However, because of the risk of side effects, treatment may be switched to ketoconazole once symptoms have settled.

Since seborrheic dermatitis is a chronic condition with a tendency to recur after treatment is stopped, ketoconazole is primarily used for long-term management.

Precautions When Using Ketoconazole | Pregnant or Breastfeeding Women Should Consult a Doctor

The use of ketoconazole is not recommended for women who are pregnant.

Although ketoconazole is minimally absorbed through the skin, teratogenic effects have been reported in animal studies following oral administration.

Breastfeeding women should also exercise caution with ketoconazole, and its use should only be considered when the clinical benefit to the patient is judged to outweigh the potential risk.

Side Effects of Ketoconazole | Itching, Dryness, Irritation, Redness, and More

The following side effects have been reported with ketoconazole:

  • Itching
  • Dryness
  • Contact dermatitis (irritation/rash)
  • Redness
  • Stinging or burning sensation
  • Swelling (edema)

The frequency of each side effect has not been fully established, but side effects associated with ketoconazole are relatively uncommon.

Is Ketoconazole Available Over the Counter? It Cannot Be Purchased at Drugstores or Online

Ketoconazole is a prescription medication. It is dispensed only under a doctor’s prescription and is not available over the counter.

It is not sold at pharmacies, and cannot be purchased through online retailers such as Amazon or Rakuten.

Some online retailers offer ketoconazole sourced through personal importation from abroad. However, personal importation carries the risk of receiving a product containing ingredients that differ from what is described, and is therefore not advisable.

Ketoconazole — A Topical Medication with Potential Benefits for Seborrheic Dermatitis and AGA

Ketoconazole is a prescription medication that may help eliminate fungi.

It may be beneficial in the treatment of seborrheic dermatitis, candidiasis, and tinea. It also has the potential to inhibit 5α-reductase activity, which may offer benefits in AGA treatment.

However, ketoconazole does not promote new hair growth or stimulate hair regrowth. If you are seeking to address hair thinning, consider combining ketoconazole with a hair loss treatment containing finasteride or minoxidil.

For detailed information on finasteride and minoxidil tablets, please refer to the respective pages below.
